Description:
Job Summary:

The structural heart coordinator will play an integral role in the growth of the structural heart program at Kaiser mid-Atlantic. The coordinator will provide care to patients through Cardiology Services and Structural Heart Disease program. This role serves as a primary liaison for all disciplines and services interfacing with the Structural Heart Program. The coordinator monitors patient outcomes and initiates/participates in quality improvement activities using advanced data analytics and best practice. The coordinator will interact with patients to guide them through the work-up, procedure, and post-procedure care related to structural heart procedures including but not limited to transcatheter aortic valve replacement (TAVR), percutaneous left atrial appendage closure (Watchman), and percutaneous mitral valve edge to edge repair (Mitraclip). The coordinator will help some of most complex patients at Kaiser to receive life-saving treatments. The coordinator will also work to improve the coordination and workflow for structural heart patients as well as help roll-out new procedures and technologies to our patients as they become available. The coordinator will evaluate modern and more streamlined approaches to patient care based on the latest technology and practice. The coordinator will be directly involved in improving the way care is provided to structural heart patients at Kaiser.

Essential Responsibilities:


  • Serves as the primary coordinator for care of structural heart patients.

  • Serves as the direct contact with patients undergoing TAVR and other structural heart procedure evaluations, after they have had initial contact with a member of the structural heart team.

  • Schedules and coordinates the patient evaluations for structural heart procedures, including coordinating one-day evaluations with testing at both Kaiser facilities and external facilities.

  • Implements and monitors patient care plans for structural heart patients. Monitors, records, and communicates patient condition as appropriate.

  • Communicates with the structural heart team and other health care providers including cardiac surgery, anesthesia, non-invasive cardiac imaging, radiology, and other health professionals to deliver optimal patient care.

  • Coordinates, delegates, and schedules patient referrals and appointments for structural heart procedures

  • Meets with the director of the structural heart disease program at least weekly to discuss patients actively undergoing structural heart procedure evaluations.

  • Maintains excel spreadsheets with details of every patient undergoing evaluation for each of the major structural heart procedures, including but not limited to (TAVR, Mitraclip, Watchman) and updates it regularly.

  • Assists in the development and delivery of education for staff regarding structural heart pathologies, procedures, and protocols.

  • Takes a role in the evaluation of the effectiveness of interventions in the structural heart program.

  • Participates in the evaluation of procedures, and patient standards. Identifies performance improvement opportunities. Serves as a resource and positive proactive leader for the department.

  • Maintains and updates communication materials including power-point files and spreadsheets for weekly structural heart team meetings and ensures these materials are disseminated to all team members.

  • Prepares the materials including power-point presentations for each structural heart case.

  • Reviews documentation for patients to ensure that all co-morbidities and details of their history are adequately captured.

  • Coordinates required post-procedure appointments and imaging across the mid-Atlantic region for each patient.

  • Follows each patient post-structural heart procedure and checks in with each patient at pre-specified times to ensure that all required registry data fields are captured for each patient in a time and efficient manner.

  • Responsible for obtaining and maintaining records required for registry databases.

  • Responsible for entering and updating data in registry databases to ensure compliance.

  • Prepares quarterly comprehensive reviews of program data from each of the structural heart related registry databases and reviews program outcomes with the director of the structural heart team.

  • Serves as direct contact for patients post-TAVR or other structural heart procedures.


    • Performs other duties as assigned.
    Basic Qualifications:
    Experience




    • Minimum three (3) years with background or experience required in Cardiac Cath/Interventional, Electrophysiology, CCRN, RCIS or ACLS

    Education

    • Bachelors degree in Nursing (BSN).
    License, Certification, Registration
    • This job requires credentials from multiple states. Credentials from the primary work state are required before hire. Additional Credentials from the secondary work state(s) are required post hire.
    • Registered Nurse License (Maryland) within 6 months of hire AND Registered Nurse License (Virginia) within 6 months of hire
          OR
    • Compact License: Registered Nurse within 6 months of hire
    • Registered Nurse License (District of Columbia) within 6 months of hire
    • Basic Life Support from American Heart Association
    • Advanced Cardiac Life Support
    Additional Requirements:

    • Electrophysiology, CCRN, RCIS or ACLS
    • Experience with Structural Heart (TAVR, Valve Replacement)
    • Advance use of Excel and data analytics.
    • Experience in MS Office to include Excel and Outlook
    Preferred Qualifications:

    • Minimum of three (3) years previous experience in cardiology is preferred.
    • Master of Science in Nursing or a related field.
