Be able to monitor the medication therapy of inpatient ambulatory patients as assigned based on the understanding of patient variables such as age, gender, and other physical characteristics. Confers with and assists medical personnel in the care and treatment of these patients; provides medication information; recommend dosage and formulary changes; Uses aseptic technique while compounding sterile medications in a glove box; monitors medication distribution; dispenses and distributes pharmacy products; manages all aspects of Pyxis stations. Promotes high quality, evidence-based, cost-effective pharmaceutical care. Performs these duties in the clinical areas assigned and other duties as required. The Pharmacist has the above listed responsibilities to ensure Kaiser continues to provide quality care and service excellence.
- Monitors the medication therapy of inpatient ambulatory patients to assure optimum therapeutic results and cost-effective prescribing. Review Patients progress to detect adverse drug reactions, and confers with, and assists medical personnel in the treatment of patients. Interviews patients to obtain information regarding medication use, medication allergies and sensitivities. Documents the information in the patients medical record when appropriate. Advises patients of directions for use, medication storage requirements, importance of compliance, precautions and warnings for medication therapy. Provides medical personnel with medication information. Makes recommendations on medication orders and dosage adjustments. Monitors the effectiveness and accuracy of institutional medication distribution. Reviews physician orders and verifies accuracy of transcription data entry into pharmacy computer system. Verifies accuracy of medication preparation and dispensing by pharmaceutical personnel. Reports discrepancies to appropriate medical and/or administrative personnel. Responds to medical emergencies according to established procedures. Supports the ACC/CDU and procedure suite patients. Completes medication reconciliations for assigned patients. Provides discharge medications afterhours. Manages all aspects of the Pyxis machines. Upholds Kaiser Permanentes Policies and Procedures, Principles of Responsibilities and applicable state, federal and local laws.

- Pharm D or Bachelors of science in pharmacy.
- Pharmacist License (Georgia) required at hire
- Basic Life Support
- Possess comprehensive knowledge of current pharmacological/biopharmaceutical principles, medical terminology, pathologies and disease states, and the ability to relate this information to the patients medical management plan.
- Working knowledge of aseptic technique and the ability to prepare sterile IV products. Excellent verbal and written communication skills.
- Knowledge of Federal and State laws and regulations regarding the provision of pharmaceutical services.
- Ability to extract pertinent information from the patient, or other sources, and relate it to the patients medical management plan.
- Ability to type medication labels.
- Ability to teach and evaluate the effectiveness of teaching.
- Comprehensive knowledge of emergency medications, and the ability to become CPR-certified. Consistently demonstrates the knowledge, skills, abilities, and behaviors necessary to provide superior and culturally sensitive service to each other, our members, contracted providers and vendors.
- Minimum one (1) year of hospital experience including compounding sterile products.
